If you are a doctor, nurse, midwife, teacher or police officer you can lodge a verbal mandatory report by calling the Department for Child Protection’s Mandatory Reporting Service on free call 1800 708 704.

However, by law, you must also lodge a written report.  Please provide as much information as possible, as this will assist with the assessment of the child’s safety.

Alternatively, you can complete the Mandatory Report Form, which is available in PDF (click here) format. Print the form for completion, and send it to the Mandatory Reporting Service via fax or post.

Mandatory reporters, your own organisation may have a customized mandatory reporting form in hard copy or online.

Non-mandatory reporters
If you are not a mandatory reporter, but you believe that a child is suffering child sexual abuse, or any other form of abuse (emotional, psychological, physical or neglect), please contact your local Department for Child Protection
District Office.
